*Problems playing during group

We keep getting this notation on Keegan's papers from school. I always get them a day later. So Tuesday morning, I got Monday's paper. Had the notation. I talked to Keegan privately at school and informed him that I did not expect to see this on tomorrow's page. This morning I drop Keegan off and pick up his paper for Tuesday. Again, the notation. This morning I informed Keegan that he lost TV for the day. Today when I picked him up from school, I asked if he paid attention during group. He said, ''YES!! But there might be a problem during story time..."
